Top claim denials and rejected as unprocessable

Did you know?
A claim adjustment reason code (CARC) and a group code on your remittance advice describes why a claim or service line was paid differently than it was billed and who is responsible for the adjusted amounts. Visit the Washington Publishing Company external link for a complete list of the CARC codes and group codes.
Please review the FAQs and other resources listed below to avoid common denials and claims rejected as unprocessable. Billing Medicare correctly the first time increases your cash flow while reducing provider burden.

Once a claim is processed, Medicare decides to either pay or deny. However, in some situations, a decision to pay or deny isn’t possible because the claim has billing errors. First Coast rejects these claims as unprocessable for you to correct and resubmit.
