Submission of new drug information

First Coast appreciates your efforts to provide us with information regarding new drugs, new or revised indications, or marketing materials for forthcoming drugs or medical devices. Specifically, we are looking for the wholesale acquisition cost (WAC), average wholesale price (AWP), national drug code (NDC) number, indications for use, and details about any off-label uses.

It is not necessary to submit your information to multiple people at First Coast. Information submitted to medical affairs will be kept on file and disseminated accordingly.
Once received, the information will be forwarded to the appropriate staff for reference in the event future claims are submitted. Submission of your information does not guarantee coverage by First Coast. Section 1862(a)(1)(A) of the SSA (Social Security Act), reasonable and necessary guidelines, applies to all services reported to Medicare.
We will contact you if we have any questions regarding your submitted information.

Information regarding new drugs, indications, or marketing information should be submitted as indicated above. The open meeting is designated for the presentation of information related to proposed LCDs (Local Coverage Determinations) only.
Note: HIPAA regulations prevent us from discussing providers’ claims with a third party. Providers should contact our customer service with any questions regarding claim denials, pricing and/or billing.
