This reason code W7088 is assigned when a claim is submitted without a specific HCPCS payment code, G0466, G0467, G0468, G0469, or G0470. 

Federally qualified health centers (FQHC) shall use a specific HCPCS payment code when submitting a claim for FQHC services payable under the FQHC prospective payment system.
