Digital Mental Health Treatment Devices
Medicare will reimburse DMHT devices provided incident to professional Behavioral Health Services used along with ongoing treatment under a behavioral health treatment plan of care. A physician or other practitioner who's authorized to diagnose, evaluate and treat a mental health disorder may prescribe or order a DMHT device. Treating practitioners must diagnose patients, prescribe or order DMHT device and incur cost of providing DMHT device to patients as a supply. Patients actively use DMHT devices at home or in an outpatient setting.
Procedure Codes
- G0552 - Supply of DMHT device and initial education and onboarding, per course of treatment augmenting a behavioral therapy plan
- G0553 - First 20 minutes of monthly treatment management services directly related to the patient's therapeutic use of the DMHT device augmenting a behavioral therapy plan, physician or other Qualified Health Professional (QHP) time reviewing information related to use of the DMHT device:
- Including patient observations and patient specific inputs in a calendar month and requiring at least 1 interactive communication with the patient or caregiver during the calendar month
- G0554 - Each additional 20 minutes of monthly treatment management services directly related to the patient's therapeutic use of the DMHT device augmenting a behavioral therapy plan, physician or other QHP time reviewing information related to use of the DMHT device:
- Including patient observations and patient specific inputs in a calendar month and requiring at least 1 interactive communication with the patient or caregiver during the calendar month
- List separately in addition to HCPCS code G0553
Conditions for G0552
- Device Classification
- The DMHT device furnished to the patient must be cleared under section 510(k) of the Federal Food, Drug, and Cometic Act, or granted De Novo marketing authorization by the Food and Drug Administration (FDA) and must be classified under 21 CFR 882.5801, Computerized Behavioral Therapy Device for Psychiatric Disorders or beginning January 1, 2026, under 21 CFR 882.5803, Digital Therapy Device for Attention Deficit Hyperactivity Disorder. FDA clearance or authorization under a different device classification does not satisfy this requirement.
- Billing Practitioner Incurs the Cost
- The billing practitioner must incur the cost of acquiring and furnishing the DMHT device to the beneficiary as a supply. MACs shall not pay G0552 when the billing practitioner incurs no cost in furnishing the device, or when the beneficiary independently procures the device.
- Incident To Ongoing Treatment
- The DMHT device must be furnished incident to the billing practitioner's professional services, in association with ongoing behavioral health treatment under a behavioral health treatment plan of care.
- Prescription/Order
- The billing practitioner must prescribe or order the DMHT device, but need not be the one who originally made the mental health condition diagnosis.
- Diagnosis
- Payment may only be made for DHMT devices for mental health treatment in accordance with the use indicated under their FDA classification under 21 CFR 882.5801, Computerized Behavioral Therapy Device for Psychiatric Disorders or 21 CFR 882.5803, Digital Therapy Device for Attention Deficit Hyperactivity Disorder.
- Practitioner Scope of Practice
- The billing practitioner must be a physician or other qualified non-physician practitioner legally authorized, under applicable state prescriptive authority and the device's FDA clearance or authorization, to diagnose, evaluate, and treat the mental health disorder, and to prescribe or order the DMHT device.
Incident to Provision
Incident to a physician’s professional services means the services or supplies are furnished as an integral, although incidental, part of the physician’s personal professional services in the course of diagnosis or treatment of an injury or illness.
A physician, CP, CNS, NP, PA, or CNM may have outpatient psychiatric services and supplies furnished incident to his or her professional service. To be covered under the Incident to provision, the following requirements must be met:
- The services and supplies must be an integral part of the patient’s normal course of treatment during which the physician or other listed NPP has personally performed an initial service and remains actively involved in the course of treatment.
- The services and supplies are commonly furnished without charge (included in the physician’s or other listed NPP’s bill).
- The services and supplies are an expense to the physician or other listed NPP.
- The services and supplies are commonly furnished in the physician’s or other listed NPP’s office or clinic; and
- The physician or other listed NPP provides direct supervision, which means he or she is present in the office suite and immediately available if needed.
Note: Under the MPFS, it is not payable for physicians or NPPs to provide incident to services in an institutional setting (hospital or SNF).
Behavioral health services provided by auxiliary personnel are exempt from the direct supervision requirement for incident to services. Incident to behavioral health services can be provided under the general supervision of a physician or an NPP. Under general supervision, the physician or NPP may be contacted by phone, if necessary, as the physician’s or NPP’s presence isn’t required during the service.
Services and supplies furnished by CPs, CSWs, CNSs, NPs, PAs, CNMs, MHCs and MFTs may also be covered when furnished as an incident to the professional services of a physician or other specified NPP if they would have been covered when furnished incident to the services of a MD or a DO.
