Request a new LCD or an LCD reconsideration electronically
Are you using regular mail for submissions to First Coast? We have several electronic options that are easier and faster for you to use. Additionally, submitting documentation electronically:
- Saves time, electronic submission eliminates mailing delays
- Eliminates administrative costs, electronic submission cost nothing
- Protects patient information, electronic submission is secure
- Allows for the immediate confirmation of receipt, electronic submission provides transaction confirmations

Request a new LCD
A new LCD request process is a mechanism by which interested parties within a contractor’s jurisdiction can request the development of a new local coverage policy. If the attachment size for clinical citations exceeds 10 MB, the requestor must send the articles and supporting documents via multiple, smaller e-mails. |

LCD reconsideration request
The LCD reconsideration request is a process by which an interested party requests modification of an existing, active LCD. For more information, including recommended information to include in the request, review the LCD reconsideration process.
