The Care Management workshop series supports providers in staying connected with Medicare-covered care coordination services. The first webinar of this four-part series will review chronic care management (CCM) and principal care management (PCM) services, which support patients with chronic conditions. We will define each service, outline patient and provider eligibility and review billing and coverage requirements.
