Submit provider enrollment applications electronically

Are you using regular mail for submissions to First Coast? We have several electronic options that are easier and faster for you to use. Additionally, submitting documentation electronically: 

  • Saves time, electronic submission eliminates mailing delays
  • Eliminates administrative costs, electronic submission costs nothing
  • Protects patient information, electronic submission is secure
  • Allows for the immediate confirmation of receipt, electronic submission provides transaction confirmations

Initial enrollment 

In general, a provider or supplier shall enroll as an initial applicant if:

  • Initially enrolling in the Medicare program or enrolling as a provider or supplier in a new geographic jurisdiction.
  • Seeking to reestablish itself in the Medicare program after a voluntary withdrawal from the Medicare program, or subsequent to a termination or revocation of enrollment based upon any CMS authority under Title 42 of the Code of Federal Regulations (CFR).

PECOS

PECOS supports the Medicare provider and supplier enrollment process by allowing registered users to securely and electronically submit and manage Medicare enrollment information online.

You must have an account with the Identity and Access (I&A) Management System prior to submitting provider enrollment applications in PECOS.

To perform an initial enrollment in PECOS, select:

  • My Associates
  • Create Initial Enrollment Application

Provider Enrollment Gateway

The Provider Enrollment Gateway is a free self-service tool that does not require enrollment.

The Provider Enrollment Gateway can be used as a secure mechanism for providers and suppliers:

  • Uploading paper enrollment applications
  • Submitting responses to development requests for a paper application submitted through the gateway
  • Requesting a copy of enrollment correspondence for a paper application submitted through the gateway
  • To check the status of a previously uploaded paper application submitted through the gateway

Change of information 

Providers and suppliers use the CMS enrollment application process to report changes of information as required to remain in compliance with the requirements to participate in Medicare.

Revalidation 

Providers and suppliers use the CMS enrollment application process to revalidate their Medicare enrollment record. Providers and suppliers are required to revalidate every five years.
