Revalidation overview
CMS publishes a national database of all providers / suppliers and their revalidation due date.
The database uses a search function to view a provider's / supplier's revalidation due date. You may search by:
- Organization name
- First/last name
- NPI
- Location
Revalidation applications must be received by the due date listed in the revalidation letter. Failure to submit the revalidation application by the due date, can result in a "stay of enrollment".
The stay of enrollment notification will be sent indicating the time frame we must receive the revalidation application:
- The provider / supplier has 30 days to submit the revalidation application and during this timeframe, claims will reject.
Failure to respond to the stay of enrollment revalidation request by the due date will result in being deactivated:
- Deactivated status may result in a gap in coverage (no payments) between the date of deactivation and the new Medicare effective date.
- Note: That the deactivation effective date is retroactive to the date of the non-compliance, or the date by which the provider / supplier was required to submit the revalidation application to CMS (the revalidation due date).

If you decide to mail the application, you need to mail the completed, signed form and all supporting documentation to us.
