New provider roadmap: Provider enrollment 

Provider enrollment is the process of credentialing a provider and supplier to determine eligibility for enrollment into the Medicare program. Providers and suppliers must be enrolled in Medicare to render services to beneficiaries and receive reimbursement. 

Step 1: Choose submission method

There are three ways to submit your provider enrollment application(s):

Step 2: Complete application and supporting documentation

If you choose to submit your application online through PECOS, the application process will start by asking a few questions about your specific enrollment or business structure scenario. Your responses to the questions will automatically determine the required sections to be completed on the enrollment form.

There are multiple CMS Medicare enrollment applications. Your required application(s) will vary depending on your enrollment/business structure scenario. To learn which paper application(s) are required, access our:

This tool will ask specific questions regarding your potential enrollment and in turn will specify which application(s) need completed.

We also have tutorials on our website to assist with filling out application(s). The tutorials review each section of the application.

Step 3: Submit application and track status

The next step is submitting your application and supporting documentation. 

If you're completing your application via PECOS, make sure to select “complete submission”

Note: To avoid potential processing delays, please sign the application before submission.

Once the application is submitted, you can track the status:

  • If the application was completed via PECOS, the fastest way to track the status is through PECOS
  • You may also track the status of PECOS and paper-based applications by accessing the Provider Enrollment Status Tool. Please allow two weeks for the application(s) to be uploaded into our system.

If we need additional information, a development letter will be sent to the contact person listed on your enrollment application. It is vital that you submit the requested information within the development letter.

  • If application is submitted via PECOS, the contact person will receive the development letter via email. If the application is submitted via mail, the contact person will receive the development letter in the order below:
    • Fax (if provided on application)
    • Email (if provided on application)
    • Mail

Note: There is a 30-day development window from the date on the letter. The quicker we receive the requested information, the faster we can process your application.

Step 4: Receive approval

Once the application has been approved, the contact person listed on the enrollment application will receive the approval letter via email, mail or fax.
