Has there been a delay in the opening of your practice location?

An effective date is required for a practice location on an enrollment application. This date should reflect when the entity plans to start or started seeing patients at the practice location. If you submit an application and are experiencing delays that will impact the effective date (e.g., construction delays), it is imperative that you contact us. 

At times, a site visit is required to complete an enrollment application. If we perform a site visit on a practice location that has been delayed, this could result in the denial of your enrollment application and impact your potential effective date. If you are experiencing delays with the opening of your practice location, you have two options:

  • Request the application be withdrawn until you are closer to the practice location opening date (best choice for extreme delays)

  • Request to change the effective date on the application

Note: We do not accept applications if the effective date is more than 60 days in the future.
