Revised coversheets are now available for PAR OPD submissions: Learn about the new fields to avoid delays

Effective December 9, First Coast updated both the standard and expedited Prior Authorization Request (PAR) coversheets with new fields to improve the submission and response processes. These fields include selection of modifiers RT, LT or 50, site levels, alternative contact information and a comment field for detailed information related to medical review.

You can access the forms at First Coast’s prior authorization program for certain hospital outpatient department services webpage, along with helpful instructions for completing the PAR coversheets for hospital outpatient department (OPD) services.

Please note that it’s important that all fields on the coversheet are completed to avoid delays in the process.
