Welcome to First Coast Service Options’ (First Coast) online provider enrollment application status lookup. Choose from the buttons below to select the type of enrollment status you are interested in checking. For help with this tool visit ourEnrollment status lookup: Help guide
Here is the information you need to provide in order to search application status:
  • All active applications: Check all active applications in process; this requires your NPI or PTAN
  • Single application – Review a particular application; this requires a DCN (document control number) or CCN (correspondence control number)
  • Revalidation letter – Confirm if you have been sent a revalidation request; this requires your NPI or PTAN.
  • PECOS Tracking ID - Check the status of applications submitted through Internet-based PECOS; you need a tracking ID number.

Please enter your PTAN or NPI to obtain a list of pending submissions currently in process.
Please note: Your application status will be available within 15 days of the date it is received by First Coast.
