Updated status indicator for Q5122

CMS updated the status indicator for HCPCS code Q5122 (Injection, pegfilgrastim-apgf, biosimilar, (nyvepris), 0.5 mg) from “E2” to “K” for dates of service between January 1 and March 31. This change was included in the April 2021 IOCE software.
For HCPCS code Q5122, First Coast updated the status indicator to “K” and the January 2021 OPPS payment quarterly rate to $366.89 and the January 2021 OPPS 340B payment rate to $253.48.

First Coast will adjust claims when brought to our attention. If you think your claim was impacted, please contact customer service to request claims with dates of service January 1 through March 31, for HCPCS code Q5122 to be reprocessed.
