Request access to SPOT FAQ

Quick tips for requesting access to SPOT

1. Review your provider's information in internet-based PECOS. You will be required to enter the billing entity’s NPI, PTAN, last five digits of the TIN, the practice official’s name and role as designated in the billing entity’s official Medicare enrollment record. If any of the information does not match the official record, the application will be denied.
2. EIDM and SPOT accounts may not be shared. Each account belongs to the individual.
3. Website viewing recommendations: Use the latest available version of Internet Explorer or other browser of choice and be sure compatibility view is turned off.
4. Please take your time answering remote identity verification (RIDP) questions from Experian. Incorrect answers will only delay your access to SPOT. Experian considers RIDP inquiries as “soft inquiries” that do not impact your credit report positively or negatively.
Q: I already have an Enterprise Identity Management (EIDM) portal account. How do I request access to SPOT?
A: After you have established your EIDM account, you may request access to the SPOT application. Before you begin, please verify the billing provider’s official enrollment record in internet-based PECOS external link. The information submitted in your application will be compared to the corresponding information in the PECOS enrollment record external link.
Note: If you require assistance during the application process, you may email or call 855-416-4199.
Follow the instructions below to complete your SPOT registration and gain access to the SPOT application through the EIDM portal.

Part II – Request access to SPOT

If you do not have an EIDM account, please view the complete registration instructions. If you are already logged into the EIDM portal and your identity has been verified through Remote Identification Proofing, skip to Step 4 of the process.

Enter your UserID and Password and click Login

2. My Apps

Click Request/Add Apps

3. Type “SPOT” in the search field next to ‘Access Catalog’

Click in the window ‘Start typing to filter apps…
Click on the ‘Show All’ button next to the search field.

4. Click the ‘Request Access’ button.

Click ‘Request Access’ then click at the bottom of the SPOT’s catalog entry.
If you need assistance during the process, the SPOT Help Desk contact information appears above the ‘Request Access’ button.

5. Select ‘FCSO Portal User’ from the ‘Select a Role’ drop-down menu.

The SPOT will automatically be listed in the Application Description drop-down menu; however, you must select the proper role.
Do not select any other role.
Note: If you select the incorrect role (i.e., FCSO Help Desk User), your application will be denied, and you will need to begin the registration process from Step 1.

6. Review ‘Identify Verification’ information

Although your EIDM account has been created, you must wait at least five minutes beginning the process for requesting access to SPOT.
The next steps will involve verifying the information you entered in steps one through five through information databases.
This process is known as Remote Identity Proofing (RIDP) external pdf file.
If you have established accounts through other EIDM hosted applications such as Provider statistical and reimbursement (PS&R) or PQRS, then you will not have to repeat RIDP.
Click ‘Next’

7. Review information about ‘Terms and Conditions

The Experian identity verification service is used by CMS to confirm your identity when you need to access a protected CMS Application.
If you have not previously been through CMS Enterprise Portal’s required identity verification, you may be prompted for RIDP when requesting access to SPOT.
You will be asked to provide a set of core credentials, which include full name, current address, residential phone number, and social security number.
The Experian identity verification service will use your core credentials to locate your personal information in the Experian database and generate a set of questions.

8. Enter ‘Your Information’ in each of the blank spaces.

Full Legal Name
Social Security Number
Date of Birth
Current Residential Address
Personal Phone Number
You must use your full legal name. Refer to your Driver’s License or financial account information.
Your surname and other information such as date of birth has to match the information Experian has for you on file.
Do not use nicknames.
If you have a two-part name, enter the second part in the middle name field. (i.e., Mary Beth would have Mary in the first name field and Beth in the middle name field)
Enter your current residential address:
Address where you receive financial statements including credit cards and/or utilities
Address you most consistently use for billing purposes
Address associated with your credit report
If you have a recent change in address, you can try to ID proof with a prior address.
Click ‘Next’

9. Complete the ‘Verify Information’ questions.

Click on the radio button next to each correct answer
Click ‘Next

10. If your information is verified...

Congratulations, you have completed the RIDP process.
Click ‘Next’
Proceed to Step 15, scroll down to view further instructions on requesting access to SPOT.

11. If your information cannot be verified…

In the event your information cannot be verified through RIDP, you will be asked to contact Experian Verification Support Services.
You may return to this application once you have resolved the issue with the Experian database.

12. Contacting Experian

When you contact Experian Verification Support Services, please keep your Review Reference Number close at hand.
If Experian is able to resolve the information, login to your EIDM account and begin the process of requesting access to SPOT as illustrated in Step 4.
When you arrive at Step 12 again, the ‘Your Information’ box will include a checkbox to confirm that you have contacted Experian and they have resolved the issue with your individual information.
Once you enter your information, the CMS EIDM system will verify your contact with Experian.
If Experian successfully verified your identity, you will be able to complete the SPOT access request form.
If Experian is unable to verify your identity, contact the SPOT Help Desk and request to be manually verified.

13. Complete the ‘Request New Application Access’ form.

The Name subsection of the application will be prepopulated with your full name and the last four digits of your SSN.
The only editable fields in the Name subsection will be the Title drop-down menu and the Professional Credentials field.
Please enter your organization’s name and address in the Business Contact Information subsection
If your organization’s address is located outside of First Coast’s jurisdiction (i.e., Florida, Puerto Rico, or the U.S. Virgin Islands) and you represent a provider within First Coast’s jurisdiction, you must use the provider’s business address. However, you may list your organization’s number in the Office Phone Number field.

Click the Next button to proceed to the next page of the application:

14. Complete the information regarding your billing entity.

Enter the billing entity’s NPI, PTAN, last five digit of the TIN, and the practice official’s name and role as designated in the billing entity’s official enrollment record in internet-based PECOS.
that bill Medicare as a group, professional association, or corporation, should enter the group, professional association, or corporation’s NPI/PTAN/TIN and practice official listed in the official PECOS enrollment record for that organization.
a provider bills Medicare under his or her own personal SSN/TIN, then the individual practitioner’s provider-specific information should be used.
Note: When you submit your application, the information submitted is compared to the official enrollment record in PECOS. If any of the information does not match the official record, including the role of the practice official, the application must be denied.
Enter the reason for your access request (e.g., Access to Medicare data)
Click the Next button.

15. Review your information.

If all of the information is correct and reflects exactly how it appears in PECOS external link, please click the Submit button.
Note: If you need to edit any information, click the Edit button, and the application will make all fields, except your name and SSN, editable.
Once you have completed your corrections, click the Save button, which will return you to the Review screen.

16. Save your tracking number

The Request New Application Access Acknowledgement page will appear.
Make note of your tracking number (e.g., 167539), and click the OK button.

17. Confirmation email

You will receive a confirmation email also containing the information provided on the acknowledgement page.

18. Completion and approval

The authentication and approval process for new users may take 5-7 business days to complete. If approved, you will receive an email similar to the one shown.
Note: If you request has been denied, you will receive an email outlining the reasons for the rejection. You will need to submit a new access request application with the corrected information.
Once your access to SPOT has been approved, login to your EIDM account and establish your multifactor authentication (MFA) device before you login to SPOT.

19. Multifactor Authentication (MFA)

Once your SPOT account has been approved, you will need to register a multifactor authentication (MFA) device to access your SPOT account.
This will be done in your EIDM profile.
Click on the | Enterprise Portal logo on the top left of your acknowledgement screen
