Plan of care or treatment signature FAQ
Q: Which specific non-physician practitioners are able to sign the plan of care or plan of treatment in a comprehensive outpatient rehabilitation facility/outpatient rehabilitation facility (CORF/ORF)?
A: Only a physician may establish a plan of care in a CORF. The written plan of treatment must be established and signed by the physician who has recently evaluated the patient. The physician must certify the initial plan of treatment or a significantly modified plan of treatment within 30 days. For additional guidance, you may review the local coverage determination (LCD) for therapy and rehabilitation services by using our LCD lookup.
