Effective date FAQ

Q: I have been enrolled with Medicare for a long time and cannot remember all of the effective dates that were submitted on my original application. What effective dates should I use on the revalidation application? Are the effective dates required to match the effective dates on the original application?
A: For these situations with revalidation applications, try to obtain the correct effective date to place on the application or get as close as possible to the original effective date. The provider enrollment analysts will attempt to work with you on the effective date submitted.
Note: There are situations where a current or accurate date is required. This would include new additions being made to the enrollment record and the certification statement. These situations should always contain the accurate date for the new addition and a current effective date for the certification statement (the date the application was signed).
