When the statement 'from' date is greater than 12/31/91, the attending physician ID and name must be present unless:
The claim is inpatient and the 1st three (3) positions of the attending ID is equal to 'SLF', then the name is not required.
The claim is outpatient and the 1st three (3) positions of the attending ID is equal to 'SLF', then the name is not required.
Resources/tips to avoid or correct this return to provider (RTP) code
This RTP code is received when an attending physician is required and either the name or national provider identifier (NPI) is missing or invalid. Review the claim:
If an attending physician is required, ensure the name and NPI are identified.
If an NPI is present, ensure the physician's last name, first name and middle initial are included.
Correct the claim and resubmit.
