Provider enrollment inquiries
Status of application: To check the status of an application, providers should access the online enrollment status lookup or contact the Part B interactive voice response (IVR) system by calling 1-877-847-4992. Have the correspondence control number (CCN) of the application available when you call.
New -- Enrollment status lookup
• Check enrollment status via online status lookup
• Use the NPI and PTAN fields search for the status of all your active applications in process, or
• Use the correspondence control number (CCN) and three-digit sequence number to search for a specific application
IVR
• For "Enrollment information," within the main IVR menu - press 7; then:
• For status of an enrollment application, press 1
• The following enrollment application status information can be provided:
• Finalized -- application is denied, rejected, or returned.
• Pending -- application has been processed and is pending for approval.
• Returned development -- application is processing; however, additional information is needed.
• Approved -- application was approved and finalized.
• Merged -- application has been merged with another application.
• Enhanced enrollment options: The IVR has been enhanced with additional enrollment features. Use the following options for the information indicated:
• For a summary of applications and when to use them, press 2
• For a summary of documents required for certain specialties, press 3
• For mailing address and PECOS Internet enrollment information, press 4
• For open enrollment and participation in Medicare information, press 5
• For a summary of enrollment information available on our website, press 6
Did you know? CMS has implemented an Internet-based Medicare provider enrollment process, known as Internet-based Provider Enrollment, Chain and Ownership System (PECOS).
• Internet-based PECOS is available to physicians, non-physician practitioners, and provider and supplier organizations in all States and the District of Columbia. Internet-based PECOS allows physicians, non-physician practitioners, and provider and supplier organizations the option of enrolling, making changes in their Medicare enrollment information, viewing Medicare enrollment information, or tracking the status of their Medicare enrollment applications throughout the Internet submission process.
• Status of an Internet-based PECOS application can be obtained through the Internet, preventing the need to call the IVR.
General questions on filing applications: Several articles and helpful tip sheets explaining exactly how to complete the applications, as well as general information concerning enrollment, are available on the Provider Enrollment Web page.
• Also, a process has been established whereby calls relating to provider enrollment may be given more comprehensive service.
• Once you conduct your initial research into the matter, call Part B customer service’s toll-free line (1-866-454-9007) for additional assistance. Be sure to have the CCN of the application, as well as all copies of the application and any correspondence you may have submitted or received from FCSO available when you call.
• Note: The provider contact center no longer accepts calls concerning pending provider enrollment applications from anyone other than the contact person listed on the application.
• If a First Coast Service Options (FCSO) representative determines your provider enrollment question requires more in-depth research and assistance, he or she will assign a referral number and provide a new toll-free number for the extended service line. You must go through this process to access the extended service line.
• Call the toll-free number provided and supply the assigned referral number.
• You will then work directly with a representative from the appropriate operational area.
• Request for an enrollment application: To obtain the appropriate CMS-855 enrollment application, please refer to the Provider Enrollment Applications (CMS-855) Web page here on the FCSO Medicare provider website.
