Provider enrollment requirements inquiry

The Centers for Medicare & Medicaid Services (CMS) provides Internet-only manuals (IOM) that identify the requirements to become a participating provider in the Medicare program. FCSO also has a provider enrollment page and list serve available and provides website links directly to the CMS website for provider enrollment. Prior to contacting our customer service lines, review these resource documents and complete the appropriate forms to become a Medicare participating provider.
From the FCSO Medicare provider website, you can click on “Quick Find” and choose Provider Enrollment. This will take you to the Provider Enrollment “home page” and there you will find a box labeled “Prospective Providers." Click on the “More” button and you will see a list of questions and remarks that will assist you in becoming a Medicare provider. You can also choose the “Tips and Tutorials” box, by clicking “More” for other valuable information.
