Coding errors/modifiers inquiry
Have you received a denial due to a coding error (incorrect code, modifier, or correct coding initiative (CCI) edits)?
You should review coding guidelines to ensure you are coding correctly prior to submitting your claims. If you're receiving denials due to an invalid or incorrect code, we recommend you review the list of resources below. They explain which modifier you can submit, and what the correct coding initiative (CCI) edits are.
• CPT (American Medical Association’s [AMA] Current Procedural Terminology) -- make sure you are using the current CPT coding book. The CPT book is available from a variety of sources, including the AMA (www.ama-assn.org
).
• HCPCS (Healthcare Common Procedure Coding System Medicare’s National Level II Codes) -- make sure you are using the current HCPCS book. Books are available at (www.amabookstore.com
) or call 1-800 621-8335.
• NCD/LCD -- First Coast Service Options (FCSO) has a link to all national coverage determination and local coverage determination policies. Click the following link to go to the LCD index page here on the FCSO Medicare provider website, where you will find several links to the Centers for Medicare & Medicaid (CMS) Medicare coverage database (MCD).
• Other -- We encourage you to sign up for the FCSO eNews and CMS LISTSERV. When there is a change to a policy or coding guidelines, both FCSO and CMS post this information on the websites. Once the information is posted, you will receive email notification of the changes and a link to the corresponding article.
